This Oregon Pediatric Annex is a great example of what a completed hospital pediatric annex can look like and may be easy to adapt for a rural/community ED:

Good Morning All,
I am getting ready to complete our pediatric readiness survey with our ER medical and nursing director. The one last piece I am missing is the disaster preparedness pieces to add to our EOP. I spoke with my director and we have agreed on adding an annex document to our EOP rather that putting the pediatric specific elements in the EOP. Does any one have a similar annex document rather than me going and reinventing the wheel. As previously stated we are the ONLY rural/community based hospital within our market so none of my sister facilities would have a similar document. Thanks for reading and helping.
