    UPDATE AS OF 12/5/2025:  

    2025 Broselow Tape (36-23446 Rev. 2)-RECALLED 

    2025 Broselow Tape (36-23446 Rev. 3) CORRECT VERSION, but still has typos:

    Identified errors:
    • Under Pre-RSI Neuromuscular Blockers, vecuronium is listed as 0.1 mg/mL. It should read 0.1 mg/kg with a maximum dose of 0.2 mg.
    • Under Toxicology, flumazenil is listed as 0.1 mL/kg. It should be 0.01 mL/kg.

    How I fixed my tapes: I added correction labels directly onto each new tape using the label maker and then placed clear protective tape over them so they can be wiped clean without peeling off

    Of note, the formula on the actual color pages is also incorrect HOWEVER, the actual dose listed is correct.


    I added the following label to my tapes to ensure we knew they were fixed and if the company decided to revise them, we can easy identify if they need replaced.

    UPDATE AS OF 12/2/2025: I have received the REVISED version of the Broselow tape!  The version you want is 36-23446 Rev. 3. As long as you a purchasing through your reputable supplier (Armstrong) you will receive the correct up to date version as they do not have any of the incorrect ones in stock..

    NEW BROSELOW TAPE:

    Just a quick heads-up, the Broselow Tape has been updated again (the last version was from 2019). Last time DNV visited our facility this was one of the things they specifically checked. So, I figured I would give everyone a heads up!

    NEW UPDATES:

    Experience streamlined functionality with our newly updated tape featuring vibrant color zones, reorganized tables, and added quick-reference vitals for faster decision making. Obsolete drugs have been removed, essential updates like 3% Hypertonic Saline added, and critical airway specifications highlighted for universal clarity. This version ensures enhanced usability and reflects current guidelines.

    • Updated Guidelines: Reflects current guidelines.
    • Optimized Emergency Data: Added weights on both sides of the tape, quick-reference vitals on color zones, and modernized ISMP recommendations. Adjusted headers for streamlined resuscitation flow.
    • Refined Design: Front panel rearrangements, QR Code, "Red to Head" nomenclature, vibrant color zones, and updated AirLife logo with the new version number for 2025 updates.

    EMSC (Emergency Medical Services for Children) issued a recall notice for the 2025 Broselow tape from both Air Life and Armstrong Medical. Have they released any updates since then?

     

    RECALL: 2025 Broselow Tape

    Dear Colleagues,

     

    As you may be aware, the 2025 edition of the Broselow Pediatric Emergency Tape, manufactured under the AirLife brand (model 36-23446 rev 2), has been recalled due to incorrect information printed on the tape. This issue may impact the accuracy of weight-based dosing and equipment selection during pediatric emergencies.

     

    Airlife and Armstrong Medical have issued an official recall of the affected products. Users are advised to immediately discontinue use, quarantine any remaining stock, and follow the instructions outlined in the recall documentation.

    Greetings all - Regarding the 2025 tape, Megan is correct - The tape was recently recalled related to a few printing issues, however, I've been told that the replacements are actively being printed & should hopefully be available soon

    For the AirLife vs. Armstrong recall - Quick moral of the story - AirLife is the current "owner" of the tape, i.e. the ones responsible for revisions, etc.  Armstrong Medical is the distributor of the tape in the US.  Same tape - One company "makes it" - The other "sells & distributes it"

    I did call Armstrong today to ask if the website was selling the corrected version.   They are not planning to print a corrected version at his time.  Currently are selling version 3 with the noted errors.  Customer Service stated that while the table was incorrect - each colored section was correct.

    It sounds like certain lots have incorrect dosing of meds and joule setting s for defib and cardioversion. It is currently a class one recall for rev. 2.

    Just received our new tape and there was no notice in the shipment of noted errors with the tape. Is there any notice as to what the version 3 errors are? What table specifically is incorrect? Thanks!

    According to the thread the new tapes without errors were at the printers in June so if you just received a shipment then yours should be good. This is the version that was recalled model 36-23446 rev 2.

    There are still errors although minor on Rev 3. under the calculation basis section for vecuronium and flumazenil. This is not within the colored sections.

    On the 2025 Rev. 3 36-23446, there are still errors in the dosing calculations section. See the picture below for the highlighted errors. 

    Under Pre-RSI Neuromuscular Blockers, vecuronium is listed as 0.1mg/mL, it should state 0.1mL/kg.

    Under toxicology, flumazenil is listed as 0.1mL/kg, it should be 0.01mL/kg

    The dosing of these medications in the colored sections are correct, it is only in the reference dosing table. According to another post of this thread, Armstrong (the distributor of the Broselow tape) will not be recalling or re-printing this version.

    Please see Jennifer Cluck's post with the pictures. Although minor, v.3 also has errors with vecuronium and flumazenil. Armstrong/Air Life is not planning a recall at this time. Our organization's leaders are not happy with this response from Armstrong, but unless they recall and correct, our only option is to make note of the errors and educate our clinicians in the interim. 

    Currently, our organization is looking into a product called PediaTape. I am unfamiliar with this product but vetting and crosschecking has begun to see if this is a suitable option.
