I've worked at several facilities and each has the same rule that patients should remain in the ED 20 minutes post IM injection (with the exception of tetanus vaccine- they may leave any times after injection). Typically I have always tried to get these IM injections completed as soon as the order is in the system so that by the time you have given the injection they are about ready for d/c and that time goes by quickly, I use d/c teaching time included as part of the 20 minutes.
